Research into highlighting gaps has found that service in health and social care is extensive and that in more deprived areas health is still extremely poor and many different groups of people such as the homeless and the elderly do not use or have access to all services that are available. There are many different reasons why certain groups of people do not have access to some services these vary from social and economic changes, to being homeless.

The homeless people are the most vulnerable and the most in need of support, however, these people are the ones who find it particularly difficult to access the support and services they require.

Health care is an area in which they can struggle as they are unable to register with a GP preventing them from obtaining regular health care check-ups, leaving them to feel that presenting themselves at an acute setting such as A&E is their only option. The society we live in is constantly changing with complex health and social care needs and some of the needs are not always met.

An example of this is if you do not have a good understanding of English you may not be able to identify your needs and which services you need. Some groups in society are prioritised such as the elderly and other marginal groups.

Aid reflection

When taking part in research you need to evaluate the work you have undertaken this should include regularly updating a plan to see what changes may need to be made as you progress with your work.

When undertaking aid reflection you need to consider factors such as what could be improved and what was successful and unsuccessful about the methods of study you have chosen etc. you should also be able to reflect on the positive and negatives and make suggestions for improving your research on the topic for future studies. At this point in the aid reflection process they need to review the impact the research will have on those who have participated, they also need to consider how the research benefits others and why it is an important study. They also have to consider if they applied all ethical considerations throughout the process.

Improving practice – during research people may discover issues that need to be fixed or discover how to improve them. Improving practice is about trying to stop further tragedy’s happening such as the baby P case. An example of improving practice in health and social care is if a hospital exposed institutional abuse, here they would research into what had happened and how they could prevent it. improving practise would expose key details to how they could prevent anything like this from happening again such as policy’s that were not being followed, the practise of the hospital and the staffing level concerns. All of these can then be addressed and changed to prevent anything like this recurring.

Plan provision

– plan provision is looking at services we have in place to see if they are working effectively. It is also highlighting gaps to see where the services are going wrong. Plan provision is also putting more services in place if they are required an example of this is to help ease pressures on other services (such as A&E), to allow everyone in society to get the health and social care they need and to stop any other tragedies or incidents occurring such as staff abusing patients. An example of plan provision in health care is where resources have been put In places to help ease winter pressures by opening pop up wards in hospitals to make more beds for people being admitted from A&E, plan provision is used here as hospital chiefs will have looked at records from previous years and then considered other methods and options to help to prevent people from reaching wards.

Allow progress to be monitored

– Allow progress to be monitored is looking back at previous years to see what improvements have been made from then till now, how effective those improvements have been and then look to build upon them. Progress can be monitored via surveys and interviews (this is your primary data) or through looking at articles and results which others have found and published (this is your secondary data). Keeping up to date records, registers and data is also a vital way to monitor progress.

Monitoring progress is key when ensuring a hospital is running efficiently, monitoring is used to see how long patients remain in hospital once they are medically fit for discharge and the reason for any delays. This kind of monitoring helps to highlight the inefficiency of certain areas within the hospital such as a delay caused because a patient was waiting for a social work assessment or therapy equipment to be supplied. The results from this monitoring can then be used to improve the service and ensure that all patients are discharged in a safe and timely manner.
