A case management plan is used as a service to meet the need of clients in order to help them overcome their problems. The case management plan requires a very proactive approach to keep the client engaged (Woodside & McClam, 2016). In the report, it is discussed about a case where a 44 years-old man, who is a single father, fighting in the court of his children’s custody, and due to depression has not gained any promotion in work and earning less money. The case management plant is used here to help him come out of the situation with different assessments.
A case study is a research on the history and the present situation of the client. It is to understand the problem (Yin, 2017). The case study is about Will, a single father of age 44 who has been battling in the court with his ex-wife for their children’s custody. He is in a situation, where he has no money to pay to the court, so he will have to sell his house. He was not able to concentrate in his work and was now working on a very low wage. Will admits that he is suffering from depression and is worried about where to keep his family. His youngest son is also getting involved with bad influences. If he doesn’t sort the problem, he might lose his family again.
An assessment should be performed in order to lead the case towards a decision about what all risks are there, the strengths and the client needs (Vourlekis, 2017). According to the case study, Will was already going through a lot of issues, due to which he was going through depression. There might be chances of Will losing everything, his job, his sons if they are not taken care of properly. So the assessment in Will case was required, and it should start with collecting historical background of Will, his strengths and weaknesses and understand his needs. Because the problem can lead Will towards creating a complex situation for himself. And if required an interaction session with his family will be useful (Shinn et al., 2015).
Planning is required to achieve a goal to address the client in order to be helpful. Management of the case is an important part, as it requires interaction, involvement of stakeholders, organizations, and clients coping. It should be planned what goals are to be achieved in order to overcome the situation (Buwalda, Vissers & Swinkels, 2015).
To achieve the success of client, few strategies are to be followed. It starts with keeping the records, then planning and practice of the plan, then comes case meetings with the client and stakeholders and different organizations, then the next step to be taken is monitoring the client’s recovery (Summers, 2015).
Although there can be chances where the situation might end up being different from what was thought, so case plane will be addressed accordingly. And if no improvement is observed then the case can be transferred to other hand in the same organization for better result and action (Greene, 2017).
The support from the family a client gets or how the family background is, matters as to structure the management plan. Communication between the client and the planner is required and also with the family in order to understand the need and difficulties of the client. It will help to assess how the client thinks and how will he respond to the strategies and what will be the outcomes of his decision making.
The values of the client and the planner may differ so it may create an influence in the plan. To avoid such situation, it is important to understand Will’s values and believes, so that the plan can give a beneficial outcome.
The plan can be affected by the skills and experiences that Will and the other participants have, as every individual is skilled in different fields and have experienced difficult situations in life. It is important to focus on the skills that Will have and to prepare an affective plan accordingly, so that he can become capable of solving his problems.
Conclusion:
According to the above case study it can be concluded that, a case management plan can be useful in order to achieve a goal towards success. It is to manage a situation and help the client to fulfill his needs and requirements. It can be stated that any problem can be managed until the desired outcome is not obtained. And the case management plan can be repeated until needed for the better outcomes.
