This is the second stage to the clinical reasoning cycle, and is concerned with eliciting all kinds of information’s from the patient about his past and present medical history. In this case scenario the patient has been suffering from medical conditions like malnutrition, anorexia and extremely fatigue. Along with that the patient has also suffered a fall and has attained a number of injuries as well.
