The selection of a primary health prevention topic that will prevent the issue from causing illness in the first place will be discussed. In addition, a strategy will be developed for meeting one health need of the community that is tailored to the needs and strengths of the designated population.
The individuals of this community are facing the challenges of health issues like exposure to diesel, and air pollution. The major risk in residing in this community is due to the impact of the Chevron Oil Refinery emission as well as diesel Pm emission from the Oakland port. The regional cancer risk potential have revealed that an area of about half a million acres together with almost a populace of three million individuals possess an increased risk of around ten per million individuals (County, 2016).
The factors, which are responsible for causing illness and death in the populations of Oakland, are chronic diseases like stroke, cancer, liver disease, heart disease and Chronic Lower Respiratory Disease (CLRD). They are different health services available to them such as Medi-cal, Healthy Families Program, Child Health and Disability prevention program and Kaiser California Kids Program (Armaline et al., 2014).
The aspects of the health concerns that are not being addressed despite of the involved partners include air pollution and exposure to diesel. The government and the other firms have attempted to deal with this issue of health but till now they have not been much successful (Pope & Flanigan, 2013).
The ultimate outcomes or goals for improvement related to the health concern involve the reduction of emissions and providing a pollution-free environment to the individuals living in this community (Hoyt, 2013).
The similarities, which are evident between the data that were gathered and the data that were generated, include the issues of health and standard of living in this community. The facilities of healthcare are included in the gathered as well as the generated data. Housing occupancy, rate of population has also been included in both of them (County, 2016).

It also includes income and poverty, Educational attainment, age of the homes in the community, transportation in the community, quality as well as safety of streets and footways, neighbourhood hangouts, cleanliness of the community, along with stores and other businesses (Chowdary et al., 2015).

The weakness of this community includes the factors that are causing this particular population to become unhealthy, ill or leading them towards death. In addition to this, the different facets of the concern of health are not being tackled in spite of the attempts of the partners who are concerned to deal with their issues of health as well as other problems (Golub et al., 2013).
In this community, the areas which needs improvement includes healthcare, quality and safety of streets and sidewalks, space between the homes and businesses (County, 2016).
In this community, the health issues are mainly arsing due to the exposure to diesel and air pollution. It is among the top health concerns of this community. Hence, there is a need to prevent this issue from causing illness in the first place because this issue to a much extent has affected the populations (Hoyt, 2013). To address this concern the individuals should know the prevention regarding the harmful effects of air pollution as well as exposure to diesel. The different program of prevention of air pollution as well as exposure to diesel needs to be introduced by the government and the other authorities to address this alarming issue of health of the individuals living in this particular community. The individuals have beared the consequences of air pollution and have developed respiratory and lung diseases. If this health concern will not be taken into consideration and the primary prevention is overlooked, then the individuals will have to bear severe health issues, which could possibly lead them towards death (County, 2016).
Developing a strategy for meeting one of the health need of the community
Considering the present scenario of this community, it is essential to develop a strategy, which will meet one of the health needs of this community. The strategy involving the management of air quality should be introduced in this community. It will focus on the reduction of emissions from the Chevron Oil Refinery (Joe et al., 2014).
The goals and objectives of the project plan have been referenced from Healthy People 2020. The populations under study are facing the health issues pertaining to air pollution and exposure to diesel. The goals and objectives of this project are:
To promote health through better evaluation and treatment of the populations living in this community
To prevent the individuals from the harmful effects of air pollution, and provide them education effectively
The populations under study are prone to suffer from the respiratory diseases like asthma and Chronic Obstructive Pulmonary Disease (COPD).The goals and objectives of this project have been aligned in such a way to address the current health needs of the population of the Alameda community. The individuals living in this community will know the prevention regarding the harmful effects of air pollution and exposure to diesel (McClintock et al., 2013).
The implementation plan for implementing the activities will involve the programs of awareness and instructing the individuals regarding the benefits of this implementation plan.
The budget of this plan will be according to the funds provided by the government and other private firms (Mankiw, 2014).
This project will involve the programs of awareness and instructing the individuals regarding the benefits of the project. It will determine all the possible outcomes of this project effectively. The different program of prevention of air pollution and exposure to diesel needs to be introduced by the government and the other authorities to address this alarming issue of health of the individuals living in this particular community (Joe et al., 2014).
The teaching materials such as instruction manuals, audio-visuals and presentations will be provided to the individuals so that they can understand the prevention strategies and could remain informed regarding the ill effects of air pollution in future (Pope & Flanigan, 2013).
The individuals should be informed regarding the harmful effects of air pollution and exposure to diesel. The individuals who are affected with air pollution need to minimize the overall burden of disease related with the exposure of air pollution (Mankiw, 2014).
The government should join hands with a variety of healthcare organizations or institutions to create effective policies that will serve to enhance the conditions of health of the individuals living in this particular area. The discussed recommendations should be incorporated in the norms and regulations effectively (Joe et al., 2014).
Creating timelines for expected outcomes
This project will be accomplished in 6 months and look for volunteers who can come forward and put their efforts to execute and obtain the expected outcomes.
